USD 303 Providing Free Dental Screenings

In compliance with Kansas State Statute 72-5201, USD 303 will be participating in free dental screening for all students. These screenings will take place during the school day on Friday April 24. Attached are two forms for you to consider with regards to this free program. As the parent, you have the right to opt your child out of the screenings. To do so, you need to fill out and sign the Opt out form and return it to your child's school by April 21. Fluoride will also be offered during these screenings. Parents must consent to having fluoride used on their child's teeth. To consent, you need to check the box at the top of the fluoride form, fill out, and sign the form, and return it to your child's school by April 21.
